Terma & Syarat:

1) Tempahan melalui sistem ini adalah berdasarkan yang cepat didahulukan (first come first serve). Slot yang disediakan adalah terhad. Sekiranya tarikh dan slot masa yang dipilih telah penuh, sila buat pilihan lain;
2) Permohonan temujanji boleh dibuat seawal tiga(3) bulan sebelum dan selewat - lewatnya satu (1) hari sebelum tarikh temujanji dan tertakluk kepada kekosongan;
3)Pilih Tarikh dan Slot masa mengikut waktu operasi/bekerja rasmi berdasarkan perkhidmatan yang dipilih kerana terdapat perbezaan waktu kaunter bagi sesetengah perkhidmatan;
4) Pemohon perlu berada di klinik sekurang-kurangnya 30 minit sebelum waktu temujanji. Pihak Klinik Kesihatan Tg Malim berhak untuk membatalkan temujanji sekiranya lewat;
5) Pemohon yang didapati sengaja menyalahgunakan sistem akan disekat;
6) Pemohon hendaklah memastikan semua dokumen yang diperlukan adalah lengkap untuk tujuan penghantaran semasa temujanji;
7) Notifikasi butiran temujanji akan diemel kepada pemohon, sila tunjukkan atau cetak dan bawa semasa temujanji.
8) Pemohon adalah bertanggungjawab ke atas kesahihah maklumat yang diberikan melalui sistem. Pihak klinik kesihatan berhak membuat semakan lanjut dan pengesahan ke atas maklumat yang diberikan.

Terms & Conditions;

1) This appointment and scheduling system allocates slots on a first come, first served basis. Limited slots are available per site and there is no guarantee that a slot will always be available for a user's first choice for an appointment schedule;
2) Appointment can be made as early as three months before or at least one day before appointment date (depends on availability);
3) Please select date and time slot according official operation time/ working hours of selected service(s);
4) Please be present at least 30 minutes before the appointment time. Klinik kesihatan administration has the right to cancel late show up;
5) Users who are found to have abused the system will be blocked from securing an appointment;
6) Please ensure the documentation to be submitted is complete.
7) Notification of appointment details will be send to applicant’s e-mail, please show or print and bring it during the appointment.
8) Users accept the responsibility for supplying, checking, and verifying the accuracy and correctness of the information they provide on this system in connection with their application. Klinik kesihatan Tg Malim administration has the right to make further investigation and verification if necessary.
